This form is available online or in person at the circulation desk. Submit the completed form to the JKM circulation desk. LSTC and McCormick alumni/ae are encouraged not to discard their JKM borrowing card upon graduation. An alumnus/a may have no more than six (6) items checked out at any given time. |
