This form is available online or in person at the circulation desk. Submit the completed form to the JKM circulation desk. Before visiting any of the ACTS libraries, request an ACTS borrower card at the JKM circulation desk. An ACTS borrower card identifies you as being currently affiliated with LSTC or McCormick. Mail loans are a courtesy extended only to those living outside the Chicago metro area. No more than six (6) items may be sent via mail at any one time. |
